Transition Talks Tuesdays     

The Wisconsin Autism Transition Demonstration (WAuTD) project is partnering with the School District of Janesville for the next Transition Talks Tuesdays (TTT)! WAuTD is learning from and providing resources for families who have autistic youth 13-26 with more challenging health or social needs who need more support than other young people. TTT is a series of virtual talks on moving from teen to adult life.

TTT is designed for educators, family members, school nurses, county workers, medical workers, social workers — anyone interacting with helping teens move to adult life. All talks will be 4:00-5:00pm on select Tuesdays.

Talks will be recorded with a resource sheet developed.

Ready to Work: Finding and Keeping Integrated Employment

Students can and should connect with the Department of Vocational Rehabilitation (DVR) while they are in school. Come and learn how DVR and Supported Employment Agencies support students in preparing to work, finding work, and supporting them at work.

Health Care Transition

As we look ahead to our students/children turning 18, we need to keep an eye on health care. What steps need to be taken to move into the world of adult health care? What skills can be taught (even in middle school)? How can we tell what a student needs to know? We welcome in Hunter Markle, self-advocate, and his dad, Tim Markle, from WAuTD.

What is the purpose of a PTP?

The Post-Secondary Transition Plan (PTP) is a part of the IEP that focuses on transition. The PTP looks at education or training, employment, and independent living (including health care) goals, as well as a student’s preferences, interests, needs and strengths. It can be seen as a guide to moving from high school into adulthood. We welcome in Shana Ratzburg from the Wisconsin Transition Improvement Grant.

Getting Help as an Adult:  Aging and Disability Resource Center

Learn what services the Aging and Disability Resource Center (ADRC) provides. The ADRC has been called the “gatekeeper” to adult long-term care programs. Find out when, why, and how you will need to connect with them, whether you are eligible for long-term care or not. We welcome Jamie Dix, Information and Referral Specialist from Rock County ADRC.

Children’s Long-Term Support programs

The Children’s Long-Term Support (CLTS) Program helps children with disabilities and their families through supports and services that help children grow and live their best lives. We will learn how is eligible for CLTS and how do youth transition out of CLTS. We welcome in Angela Bouton from Rock County CTLS.

Guardianship and Supported Decision-Making
