Listed below are key transition resources for Wisconsin youth with disabilities, families, and professionals.
Members of the Wisconsin Transition Planning Coalition developed a broad list of resources available to support the transition process as it relates to education, employment, and health. We then identified Coalition members who were content experts in each of the transition focus areas (education, employment, and health). These content experts reviewed the broad list and selected the key transition resources listed below.
The resources below relate to one or more of the transition focus areas. Our Coalition has also developed a list of integrated resources. These resources support integrated transition planning that includes education, employment, and health. Some resources from the integrated resource list are also included in the key resource list below. The icon to the left indicates these integrated resources.
Academic and Career Planning Webpage – WI Department of Public Instruction
The Department of Public Instruction’s Academic and Career Planning webpage is intended to equip students and their families with the tools necessary to make more informed choices about postsecondary education, training, and careers.
Aging and Disability Resource Centers
These centers provide information on a broad range of programs and services, including long-term care. They also connect people with disabilities to appropriate supports and services.
BeforeAge18.org Website
The BeforeAge18.org website provides online resources to help teens with disabilities and their families prepare and plan for adulthood.
Developmental Disability Programs and Information – WI Department of Health Services
This webpage is designed to help those with developmental and intellectual disabilities and their caregivers find programs that meet their needs.
Division of Vocational Rehabilitation – WI Department of Workforce Development
The Division of Vocational Rehabilitation (DVR) is an employment program that assists people with disabilities find a job, keep a job, or get a better job. Businesses can work with DVR recruit & retain employees.
Family Voices of Wisconsin “What’s After High School” Training
This training covers a variety of topics related to the transition to adult life for youth with disabilities and special health care needs.
Got Transition Website
Got Transition is the federally funded national resource center on health care transition. Its aim is to improve the transition from pediatric to adult health care through the use of evidence-driven strategies.
I’m Determined Transition Guide
A transition guide to help youth and their families transition to post-secondary education and training for a self-determined future. The I’m Determined project is a state-directed project funded by the Virginia Department of Education.
Independent Living Centers (ILCs) in Wisconsin
Independent living centers (ILCs) are consumer-controlled, community-based, cross-disability, nonresidential private non-profit agencies that are designed and operated within a local community by individuals with disabilities, and provide an array of independent living services.

PACER’s National Parent Center on Transition and Employment Learning Center
The Learning Center informs parents and others about major components of transition to adulthood for youth with disabilities.
