Wisconsin Children’s Resource Centers

Wisconsin has 5 Children’s Resource Centers staffed by information and referral specialists, who are available to assist with questions about transition planning, including health benefits, changing doctors and systems from pediatrics to adult health care, independent living and employment services, and communication with schools about transition planning.
